Award Recognition Guidelines:

· No verification = no credit! It is the student’s responsibility to have his or her service hours verified in writing by an adult sponsor (one who is not a family member of the student volunteer) affiliated with the agency or work site. These hours must be turned in to the Prep Community Service Coordinator for approval. Forms to document volunteer hours can be obtained from the C.S. Coordinator and on the school’s web site.

· Deadline = April 15^th Hours credited for a school-year must be turned in by the deadline for a student to receive a certificate that year. Hours accomplished after April 15 may be credited for the next school year.

· NO double credit for community service hours Service hours needed for membership requirements in any other school or community organization cannot be credited toward a Community Service Certificate.
